European Transition Symposium
The European Symposium on Transition is an biennial conference where researchers and professionals can share their knowledge, learn, and empower each other to further improve transitional care for young people with chronic conditions or disabilities. Previously, the Symposium has been organized by Prof. Joan-Carles Suris and his team from Switzerland. He is now retired and asked us to take over.
The 4th edition focused on innovations and best practices in somatic healthcare, rehabilitation and mental healthcare.

Keynote Speakers 4th edition
Prof. dr. van Staa will be sharing her experience with the development of the Dutch Quality Standard on Transitional Care.
Prof. dr. Moons will be joining to speak about the STEPSTONES project about young people with chronic medical conditions.
Dr. Sattoe will share her experiences with patient participation in innovation and improvement of transitional care.
Prof. dr. Bratt is the project leader of the STEPSTONES project in Sweden and will be sharing the results of this project.
Prof. dr. Coyne has published about defining health care transition. She will share her latest findings.
Dr. Le Roux will be joining to speak about her work on patient participation in transitional care and research.
Prof. dr. Escher will be joining to speak about the implementation of a transition blueprint in a academic hospital.
Buijs MSc. will be sharing her experience with the development of a transition blueprint in an academic hospital.
Prof. dr. Gorter will be sharing his work on transitions of young people with chronic disabilities.
Prof. dr. Van Amelsvoort will be joining to share her work on transition in mental healthcare.
